自閉症とセロトニン

神経伝達物質のひとつであるセロトニンに関する報告が多いので話題としてまとめてみます.

そもそも事の始まりは,1961年,一部の自閉症者で血液中のセロトニンが高濃度であると報告されたことによります.この傾向は多くの研究者たちによって繰り返し確認されましたが(総説文献1,2),血中のセロトニンが高いことが,自閉症の原因に絡んでいるのか,それとも単なる結果(または関連現象)のひとつなのか結論が出ていません.加えて以下のような問題点が残っています.

  1. 増加しているという報告では,自閉症者の26-66%で増加.
  2. 血液中のセロトニン量がむしろ減少している自閉症者も多数存在する.
  3. 自閉症の傾向がない人の中でも血液中のセロトニン量が増加していることがある.
  4. 髄液中のセロトニン代謝産物(5-HIAA)の量に一定の変化がない.
  5. 他の神経伝達物質の関与も否定できない.

最近,以下のような関連報告がなされています.

1995-1997年(文献3−6):一部の成人自閉症者で選択的セロトニン再吸収阻害薬(SSRI)が有効.

1995年(文献7):血中のトリプトファン(中性アミノ酸に対する比率)が,自閉症児の血清中で少ない.

1996年(文献8):以前より報告されている自閉症に関連するHLAハプロタイプと高セロトニン血症が関連.

1996年(文献9):セロトニンの前駆アミノ酸であるトリプトファンを摂らないと多動や繰り返し動作が悪化する.

1996年(文献10,11):強迫性人格障害(obsessive-compulsive personality disorder)で,セロトニン移送蛋白(transporter)機能が症状の程度に逆相関.

1997年(文献12):セロトニン移送蛋白の遺伝子変化が自閉症に関連していることが示唆された.

1997年(文献13):PETスキャン検査で,脳局所におけるセロトニン合成を評価すると,前頭葉皮質,視床,小脳歯状核の片側性(多くは左)の合成低下が自閉症児でみられた.低下している側の反対側歯状核では逆にセロトニン合成が亢進.

1997年(文献14):行為障害(conduct disorder)において,症状に平行して血液中のセロトニン量が増加.

1997年(文献15):セロトニン受容体の二次伝達物質(second messenger)の前駆物質であるイノシトールは,単独では自閉症に無効.

1997年(文献16):セロトニン受容体に対する抗体が自閉症児で高力価.


(解説)

選択的セロトニン再吸収阻害薬(SSRI)と呼ばれている薬の中でセルトラリン(sertraline)やパロキセチン(paroxetine)はセロトニン移送蛋白の蛋白合成を変化させずにこの蛋白の機能をブロックすることが知られており,セロトニン移送蛋白阻害剤とも呼ばれています.上記の内容をまとめてみますと:
